Who are we?
Minnesota Area Agencies on Aging and the Minnesota Pharmacists Association have joined forces to offer free medication assessments for seniors in Minnesota counties through a federal grant.

How can I receive this service?
When you attend a Medication Management Project information session in your community, you will fill out two forms. First, a health history form will help pharmacists know which conditions you are treating with medications. Second, a consent form will authorize the release of information. Any medical information will be kept strictly confidential.

A project pharmacist will meet with you one-one.
Are you taking your medication properly?

  • Drug-drug interactions
  • Drug-condition interactions
  • Appropriate doses
  • Correct drug administration and usage
  • Proper monitoring of drug therapy
  • Therapeutic duplication
  • Potential cost savings (when appropriate)
