Care Consultant Services The Care Consultant assists older adults to live as independently as possible. Consultants work with the individual and family, in person or over the phone, to identify and choose services that meet their unique circumstances. A plan will be developed for providing care, assist with problem solving, link caregivers and their loved ones to community services and offer other forms of support. |
