In-Home Help Chore and home repair services, companionship, homemaking / housekeeping, home health care, hospice, personal emergency response systems & telephone reassurance. Medicare & Other Benefits Medicare, Medicare Supplemental Insurance, Medical Assistance, Medicare Savings Programs & State Health Insurance Assistance Program (health insurance counseling). Prescription Drug Help Drug Importation Program (Minnesota RxConnect?), Patient Assistance Programs & Medicare Prescription Drug Program. Care Consultant Services The Care Consultant assists older adults to live as independently as possible. Consultants work with the individual and family, in person or over the phone, to identify and choose services that meet their unique circumstances. A plan will be developed for providing care, assist with problem solving, link caregivers and their loved ones to community services and offer other forms of support. |
