Chronic Disease Self-Management is 6-week workshop providing people with ways to deal with the social, physical and emotional challenges of chronic conditions. Develop strategies to manage them and learn techniques to deal with the frustration, fatigue, pain and isolation that often accompany these conditions. Topics include: appropriate exercise for maintaining and improving strength, flexibility, and endurance; appropriate use of medications; communicating effectively with family, friends, and health professionals; nutrition and how to evaluate new treatments. Tai Ji Quan is a falls prevention program derived from traditional Tai Chi movements and enhanced as a therapeutic practice to help older adults and those with balance disorders improve balance, coordination and postural control.
This tailored program is led by trained leaders who guide
people in learning an core eight-form routine and eight therapeutic movements from the Tai Chi Yang-style.
By the end of two 12-week sessions, participants report improved cognitive and physical functional abilities and reduced incidence of falls. Developed by Dr. Fuzhong Li, Ph.D., Oregon Research Institute.

This twice weekly (Tuesdays/Thursdays) series of classes over 12 weeks provides older adults with the opportunity to improve their balance and reduce the likelihood of falling. Research has shown that people completing the program are half as likely to fall and less fearful of falling. This workshop guides people with Type 2 diabetes to develop
lifestyle strategies to improve overall well-being. Participants learn practical techniques to manage symptoms like fatigue, 
pain, hyper/hypoglycemia and stress. 

Trained peer leaders facilitate the interactive classes encouraging development of weekly action plans and maintaining goals set. Open to any adult 18+ with diabetes or family caregivers. 

The workshop is part of the health self-management evidence-based programs developed by Stanford University and offered across the US.
